Far Exceeds Expectations: Does their job extraordinarily well AND goes above and beyond call of duty on a consistent basis. Performs duties beyond what is expected (front office helping back office and vice versa while still doing their own job excellently; example would be if at end of day after all patients have checked out, front office helps back office clean instruments so everyone can get out of office as soon as possible). Always helping out and/or taking the initiative. Is a great communicator.

Exceeds Expectations: Goes above and beyond call of duty on a semi-consistent basis.

Meets Expectations: Does their job well. May go above and beyond call of duty on an intermittent basis. Most of the time, this is where I expect people to rate others at.
